Exisiting Patients

If you are an existing patient, you can pay your bills online, view your latest health records and encounter notes, and send secure messages to the doctors and staff. If you have trouble logging in, please contact our office by phone for assistance.  

New Patients

What to Expect at Your First Visit

When you arrive at our office for your first visit, we will need to see your insurance card(s) and your driver’s license (or other valid picture identification). We will scan these into our computer system so that you will only need to present them at your first visit of each year or if your insurance changes. Unless you have already completed the new patient forms, we will ask that you complete those forms prior to seeing the doctor. You can save some time at your first visit by completing the patient forms in advance of your appointment.

 

If you have access to a printer, you can print your New Patient Paperwork, fill it out, and bring it with you to your appointment.

If you would like to fill out your paperwork online, please request an appointment first and we will send the forms to your email that can be completed on a mobile phone or computer and send to us electronically. You can request an appointment with Dr. LoPiccolo and Dr. Parrish online or by calling our office.

Once the paperwork is completed, one of our Podiatric Assistants will escort you to a treatment room where you will be asked to remove your socks and shoes from both feet, even if your complaint involves only one foot. The doctor will ask you questions about your medical history and assess your feet through an examination and some basic tests. If x-rays or ultrasound examination are needed, those can be performed in our office with results immediately available to the doctor.

If you have medical records, x-rays or other diagnostic test results which may be relevant to the reason for your visit, please bring those with you as they may assist the doctor in his assessment, diagnosis or treatment recommendations. In many cases, we may be able to begin treatment of your condition on the first visit. However, no treatment will be rendered until the doctor has discussed with you his assessment and treatment recommendations.

Prior to your visit please read our Insurance Information for an explanation of our financial policies. Our physicians and staff members have only one purpose for being here – to provide you with the best podiatric medical care available. Your feet have to last a lifetime, and we want to help you make that happen.

  • Insurance Card(s)

  • Driver’s License or other picture ID

  • New Patient Paperwork completed & signed

  • List of prescription & over-the-counter medications

  • Relevant medical records and/or x-rays if available

  • Your Feet!
